Our Company
Hope Healthcare Agency Inc. is located in Eden Prairie, Minnesota. We aim to facilitate improved developmental disability care and support to residents and clients within the area. We are direct-to-home providers of Home and Community-Based Services (HCBS), providing opportunities for Medicaid beneficiaries to get access to care services at home or in their chosen place of residence in the community rather than hospitals, care institutions, or other isolated settings. These programs aim to serve targeted population groups, such as people with physical conditions, mental illnesses, and/or developmental or intellectual disabilities.
